Sheva brachos are made on the seven days after the chasunah, often each night by a different host. The card says whom it honors, which day, where, and who invites.

The wording, line by line

Which day

Each sheva brachos is one of the seven days after the chasunah, so the day of the week and the parsha say enough; add the Hebrew date if you like.

The host

The host is often a relative or a friend of the family. He signs in his own name.
